Trouble registering e+ account

I'm trying to use a friend's address to register an e+ account so I can have some tickets sent to her house for me to pick up once I get there... but for the life of me I can not get this form to accept the address.

I can enter the postal code, city and municipality fields just fine, but when I go to enter the address (2-2-9) and apartment number (612) I just get this error message:

[番地]は[全角]で入力してください。
[マンション・アパート名・部屋番号]は[全角]で入力してください。

I've tried entering it every different way I can possibly think of, but it never works. Can anyone tell me what I'm doing wrong?

Ok, so I finally figured out how to enter the address correctly... but now I can't get it to accept any of my credit cards, at least I think that's the error message I'm getting. The odd thing about the credit card entry field is that it's asks me to enter... my birthdate? It has a pull-down menu for a month and day, and the translation I get for what the field is called is "raw month" which means nothing to me. Could it be asking me for some other date though? Or maybe e+ doesn't accept foreign credit cards anymore, which is bullshit because I had an e+ account like 5 years ago and it worked fine for me then.

Wouldn't the date field be for the expiry date of your credit card?
